With the CRT tube basically gone and Flat Screens being the norm, I read that two years ago a new SMPTE standard was introduced for title and action safety (90% and 93%). So my question is, why do the guideline in the Avid Grid overlay still look the same as they have for years...
Well, most stations are still using the old "title safe" specs.
That said, a truly cusomize able title safe would be very useful in todays world of many "standards".

I agree. Here's an NAB / SMPTE doc I found on the newer 2010 standards...
https://docs.google.com/viewer?a=v&q=cache:9MuigDY4_2UJ:www.nab.org/xert/scitech/pdfs/tv031510.pdf+title+safe+area+on+flat+screen+tv&hl=en&gl=us&pid=bl&srcid=ADGEEShRj2e-nU00ythVTkNyvAvLSSMq1c4OzfTAloUm_ONEU1saou0s3YK9yopD5d_ecC2ZfFLoSTvGucQY_nSrCK9X97F7KZ326GLGNLoCNtmF8XIxHr4cZO3kCHeOnZcWf9YSykGr&sig=AHIEtbTfwdIjTaaaO_aX75oOG_SI9LffeA
I guess my question was that if these are the new title and area safe guidelines (since 2010), why aren't they available as part of the Grid overlay options in Avid?
Are they available in any form other than the document?
In other words, has any manufacturer made use of this new standard?
SMPTE certainly sets the standard, however the industry as a whole can be slow in adapting to new standards. SMPTE's revision of the safe title standards is a good thing. It's common misconception that the entire raster is displayed with HD displays.
They certainly aren't accepted in my market.
It would be good if Avid offered better flexibility though. We have had delivery requirements of 16x9 with a protected 4x3 centre field. To do that I created my own overlay in Photoshop and dropped it into the top video layer.
It was klunky and not very convenient and a customisable overlay would have been a godsend.

The thing to remember is that SMPTE makes recommendations, not rules. No one has to do what they suggest.
So for many groups, doing nothing new is the easiest, cheapest thing to do.

Why not set your grid settings to 4:3 inside 16:9 monitor?
I will be happy when the day comes that we can completely ignore center cut safe. The restraints on using the entire canvas will be removed.
the easiest way is to customize your grid in the grid_settings: try the inset_settings - maybe 20-30px and you will get a new "outer_frame". delete title_safe and use action_safety as new title_safety.
