I've just tried upgrading my 2.66 quad4 macpro (10.5.7, QT7.6) to MC 3.5.4
What I cant do is reverse play in either source or record monitor:

CONSISTENCY CHECK FAILURE!?!: [262224]I've gone back to 3.5.1, everything is fine, except I can no longer use my command 8

I have the same error message. It is very inconsistant. Sometimes it works, sometimes I get this message.
I haven't upgraded yet but, I hope people stay on top of this to see if it's more widespread.
I am having the exact problem on two systems I upgraded today. Both Mac 10.5.7, QT 7.6. One is an Adrenaline on a 3.0GHZ Mac Pro and the other is a Mojo SDI on a 2.8GHZ Mac Pro. Connected to Unity Medianet 5.1 over fiber for storage with the project on a Unity workspace.
Tried trashing MC State, Site settings, new user, etc. Happens 10-20% of the time when trying to play in reverse in a particular project.
Tried a couple of other projects without having the problem. The client is going to try other projects tomorrow to look for a trend.
I'll try back-reving to 3.5.1 to see if this helps, but there were no complaints before the upgrade. I did do a 10.5.5 upgrade to 10.5.7 at the same time, so it could be OS 10.5.7 related.
I, too, am experiencing the same issues but on a PC. I am running Windows XP SP3, MC 3.5.4. I am connected to a 4.1.6 Unity and running MediaNet 4.1.6. I am going to investigate further and will report back with anything I find.
THis problem has been fixed and will be available for wide release in the next version.

So the workaround is to go back to 3.5.1?
The list of fixed bugs in 3.5.4 makes me scarey to do so (extreme slowness in Unity Media Network environments, installing issues with Vista 64-bit, audio scrubbing issues, audio punch in issues). In the meantine, the reverse play bug is often driving me insane.
I'd go back to (perfectly working) 3.1.x, but I'd have to demote a whole lot of sequences, and I'd lose merged BWF-M imports.
OK, so if I would be willing to give 3.5.1 a try, how do I solve the Vista 64-bit installation issue that is mentioned here:
Bug Number: UDevC00123428. Previously, the editing applications did not install correctly on Windows Vista 64-bit operating systems unless you edited the Common Files folder path. This has been fixed.
This has been fixed in 3.5.4. But that implies that I need to edit the Common Files folder path when installing 3.5.1. How and where and what do I change it to? Searched the forums, the Knowledge Base and Google, and came up with nothing.
No one? Really...?
Job - you're on the Mac side. I think when people saw Vista in your post they skipped over it. You may want to move over to the PC side to see if anyone can address that specific problem.
