I'm suprised to see 2 Myricom cards. I can't remember if that was ever supported and will have to look it up. Can't say if that is the reason the storage manager fails but I expect it isn't. Bob will know better.

Well... I think I understand what is wrong Rob but I must admit that my conclusion goes against what the profiler and documentation say. (the documentation has a crucial 'mistake' that makes this a bit difficult).
I believe that on these old ISIS 5000 chassis ISIS 4.7.x will not work under 2008 R2 SP1 storage server but only under 2008 storage server. But the only way to test this is to re-image the system with 2008 (SP2) storage server. I can provide you the image if you have lost it after formatting the servers.
I'd like to do a few more comparison checks on multiple 5000 and 5500 systems I have here in Greece. Unfortunately do not have an isis 5000 system on which I can test upgrading to 2008 R2.

Unfortunately I don't have access to this image. I might be able to create one from 5500 chassis? otherwise I will get you to link me one if possible.
The image is 13.8GB. That will require an ftp transfer or an online filetransfer service of your choice.
I can use my private ftp but my upload speed is well... low. Do you have an ftp server?

Only a specific single 10GigE Myricom NIC is supported in the ISIS Chassis. There has never been support for a 2x10GigE NIC.
I doubt that the issue is between 2008 and 2008 R2.

2008 and 2008 R2 use different Windows product key also. You can't activate R2 using "R1" product key and vice-versa.

To be absolutely clear, ISIS 5000 supports Windows Storage Server 2008 and the ISIS|5500 supports Windows Storage Server 2008 R2. There are changes made to the OS so only the image that ships with the ISIS should be used.
What exact OS is the 5000 running?

I did a remote session and his ISIS 5000 is currently running 2008 R2 SP1 which is obviously not the OS it was delivered with. But the documentation says this is ok. It basically copies the as-3000 system director requirements for equal OS level on resilient systems.
In the ISIS 5500 setup guide it writes:
When replacing or adding a second System Director in a dual System Director configuration, you must use the same operating system image on both System Directors. You have the following options:
• Reimage the newer System Director with the older Window Storage Server 2008 operating system image, to match the existing System Director and create a new Standby Filesystem on the newer System Director.
• Reimage the existing System Director with the newer Window Storage Server 2008 R2 operating system image, to match the image on the new System Director. You need to back up the metadata from the existing System Director before applying the new ghost image. Once you apply the new image to the existing System Director, restore the metadata back to that System Director. Then create a new Standby Filesystem on the new System Director.
But while the profiler and storage manager web page pass everything on the system, the storage manager cannot initialize the raid controller. Also the ISIS 4.7.11 installer immediately throws an error saying it can't. And the reason is as Rob wrote that it detects a part nr which is not the correct one.
He has removed the dual 10G card and placed the single 10G Myricom card again but that made, as expected, no difference.
As soon as I have send him the 2008 (R1) image I guess we will know.
The ISIS 5000 needs to be running Storage Server 2008. It's not possible to purchase a Storage Server license as it's considered an embedded operating system.
For a few releases there was an issue where a SD wouldn't failover if one SD was running Storage Server 2008 and the other was running Storage Server 2008 R2. In these cases, Avid could convert the Storage Server 2008 R2 license to a Storage Server 2008 license. The install had to be done by an Avid employee. Avid never sold an upgrade to Storage Server 2008 R2. Microsoft is very strict.
This has long been resolved and there's no reason to do this.
There's more to the story about this system that's not understood, it's is in a wildly unsupported configuration.

I'll try and fill in some of the missing pieces:
1) I have 2 x Problem ISIS5000 chassis that failed to update.
2) only on the one (chassis), have I tried to reinstall the OS. I tried the original image first without luck, then an image from the 5500. which is where the server is at now. I can use the image from the 5000 chassis that hasn't been touched, to reinstall the OS on this failed chassis.
3) Understood re. Dual channel card only supported on the 5500, not the 5000 chassis. The dual channel was swapped back to the single channel card.

No version of the ISIS 5000 or 5500 supported a 2x10GigE NIC. The hardware between the 5500 and 5000 is identical. The 5500 introduced the 4TB drives and the use of Storage Server 2008 R2.
Right no Dual10GB support on servers! Got it.
For some reason dual 10GB and v4.7 have stuck with me. Maybe it was the client side that supports dual 10GB in 4.7 and this is where i'm getting confused.
