Last week I started up my Mac pro to recieve an OSX error urging me to backup all data on my RAID drive. It mentioned that OSX would only grant me limited access to the drive. When I started up Avid it gave me the error "Exception: STRM_E0F." I checked the drives in disk utility and found no errors. I then ran disk warrior and rebuilt the file directory and after that I restarted and there were no more errors. I did loose some media though.Today, I received the same two errors again. I used disk warrior again and it resolved the two errors. However, I again lost more media.Disk warrior's report on the drive was this:"The original directory is damaged and it was necessary to scavenge the directory to find file and folder data. Some files that had been lost or thrown away may be recovered. It is recommended that you preview the replacement directory and examine the items listed below. All errors in the directory structure such as tree depth, header node, etc, and more have been repaired."It then had 67 pages of files claiming the file name changed. Disk warrior created two directories in the root of RAID, one called Damaged Items and the other Rescued Items. I've noticed a great deal of files called "creating354 - creating890", what do these files do?Starting up Avid it asked me about certain media files (the 67 pages worth) and gave me the option to quaritine. I quarintined them all. There located in a folder called Quarintined, what should I do with these files?I also deleted the two index files. I noticed that there is Avid MediaFiles (125gb) and OMFI Media Files (600gb). What is the difference between the two?I'm planning on buying a 1TB drive and backing up all data to it. The RAID configuration is set across 4 drives, 2x Striped + 2x Striped with both groups Mirrored. Do I have to rebuild the RAID configuration in OSX when I reformat or can I reformat with the configuration as is?Any suggestions would be greatly appreciated. I can't afford to loose any more media.

So, the OMFI directories contain the OMF media files and similarly Avid Mediafiles the MXF media. On my system, the older media is OMF and the newer stuff is MXF. By default the Avid captures media as MXF, which is the newer method.
I don't think you have to rebuild the RAID config. Just reformat. However, without knowing what is causing the problem I'd hate for it to happen again.
Before you re-do the drives, maybe talk to a tech about how they are laid out.That's an interesting setup, but remember with more drives, can come more problems when hardware goes wrong.

Yes. When you go into capture mode, and select a drive, the software "claims" the remaining drive space, so the size of those "creating" files or folders is usually equal to the free space at the time. After a successful capture, they're deleted by the MC app. So, if you have a crash during capturing, you can expect to see huge files like that. It's safe to delete them.
Your "quarantined" files may actually be just fine. What I try to do is move the unquarantined media to another folder, move the quarantined files into the "right" folder, and see if MC can read them (Ignore quarantining them on the next scan.). If they show up in MC, I might consolodate them to another drive, just for safety. If they still give you trouble, delete them. Corrupt media can cause MC to behave erratically, or to just stop working.
If DW is giving you such ominous errors, I'd suggest you back up all your media, and initialize the drive.
