So, I opened up my demo reel project that I have used for the last couple years and my bins seem to be a bit disorganized. I figure that maybe that last time I updated the reel, I was in a rush or something, who knows.
In any case, there are 2 bins that seem to have duplicate master clips pointing to the same media. But, when I delete one of the master clips (and retain the data), in about 60 seconds, the master clip magically re-appears in the bin. What's up with that??

When toolsets and bins behave erratically, it's often because site settings files have become corrupted. To rebuild, quit the app and in the settings folder delete the following: MC State, Site_Settings, Site_Attributes. On re-launch, the system will detect the missing files and build new ones.

Unfortunately, it did not work. Although I have found that it's only particular bin that seems to be affected. For example, if I have video1.mov in BinA and also in BinB, I can delete it from BinB and it stays gone... but if I delete it from BinA, it will come back every time I save.
So, for now I can work-around by moving all the clips in BinA into a newly created bin and it's fine. If I delete, the clips stay deleted. BinA is haunted or something.
Then you just had an individual bin whose settings file became corrupted. You did exactly the right thing to correct. Be sure to delete the now empty problem bin.
It's possible that in Bin A you had the Bin display set to show Reference clips. Since the sequence in the bin keeps referencing those clips, they will keep showing up.

Hoolio Edits makes a good point. Check your bin display and make sure "show reference clips" and "show rendered effects" are de-selected.
