The root cause has been found to be the ATTO UL4D driver v3.100.0.0. The latest driver v3.10.2.0 is resolving this issue.
You can download this driver update from the Knowledge Center on this site, and from the Avid DS Support Center.
Many thanks to Pierre Corbeil (Softimage) and Troy Thompson (Private Idaho) who both found this working solution the same week. Extensive testing internally at Avid, and at many volunteer facilities confirmed the new driver to be solid.

I don't believe this solution should be confined to only DS systems. This exact issue has been plaguing one of my MC Mojo SDI system since the day we bought it almost 2 years ago! We've gone so far as to replace hard drives, motherboard, full OS wipe/reinstalls to no avail.
I updated the SCSI drivers from 3.1.00 to the current version, and have gone 4 days without a lock-up!

You are absolutely correct. It is not at all a DS specific issue but rather a flaw with an old ATTO driver revision and newer HP platforms. Any ATTO drivers from release 3.10 and higher no longer show such operating system "hanging" problems.
