1) Hire an Avid ACSR to check the hardware/software setup.
2) Hire a Fiber Cable Contractor to check your cables. They have specialized meters to evaluate fiber cables.

Yes, and your warranty will be null and void and you will perish poor in the Unsupported Land.
Avid also likes to talk about 'corrupt files', please make sure you check for those.

Just hire a freelance ACSR to check for you.
Common things to check --
FC Switch (both the software config and general hardware integrity). Sometime people mistakenly connect a client in a storage port on their FC switch.
tipping of fibre (try using another port or running a direct line to the switch)
see if you have any errors from Monitor Tool
I updated one of my adrenaline in version Leopard 10.5.3, MC 3.0, Fibre Connection 5.1, driver atto, etc..
and i have aways the same problem.

I would start by doing a clean install if possible.
I had that problem for a while in two different systems (Mac and Pc). Both were the Atto announcing their own death. One day they simply stop working. Once substitued, everything was fine.
