Hello
We have had a variety of Assertion failed debug error messages in two MacPro suites over the last 18 months.
I'm now getting the following error every time I open a project in MC:
"Assertion failed: 0 <= slot && < mNumSamps File: /Snapshots/relengmin|1_1479270374/coresw/ACME/src RequestList.cpp, Line: 1120"
I've tried the following with no success:
deleted site settings
created a new user
rebuilt all Mediafile databases
unmounted local RAID
unmounted ISIS drives
uninstalled BM drivers and hardware
installed and then uninstalled AJA drivers and hardware,
reset PRAM
Erased HD and re-installed Mac OS [10.11.6] and MC [8.6.5]
Created a new project locally
I'm running out of ideas to fix this. Can anybody at Avid help?
I have downloaded reports, but like the error messages I find them unintelligible.

@nigelad: hope the weekend went well. Did you get this resolved? Past instances point to a bad MC + BMD driver combo:

What driver version were you using?
Also, were you trying to use BMD and AJA hardware on the same workstation at the same time? Or did you swap it out to troubleshoot?

I’m now getting the following error every time I open a project in MC:
To clarify: is Media Composer crashing on launch, or when opening a project? Could be an incompatible AVX Plugin.

Thanks for the responses - I took the weekend off!
The error is reliably happening each time I launch the progamme and open a project or create a new project. I can click continue and the programme doesn't crash, but the error often returns.I swapped out the hardware to troubleshoot, trying the AJA T-tap with drivers after uninstalling the BM drivers and Mini-monitor. My clean install has the AJA drivers. I'm about to try another clean install, leaving out photoshop, adobe ME, VLC, Mpeg streamclip etc.
Any more suggestions welcome.

Does the error happen with or without the BMD / AJA hardware attached?
It was occuring in both scenarios.
Since doing a fresh install of OS and MC, removing the AJA drivers seems to stop the error message.
UPDATE:
I've now installed earlier AJA drivers - 12.5 instead of 14.0.0.9 and the signs are promising.
