Make sure all cards and cables to any attached devices like I/O etc. are seated firmly. Check your Graphics card too.
Also, this has been known to occur if your deck is playing back and outputting an HD format that does not match your project type format. Example: The deck is playing back and outputting 720p, but you project is 1080i.
Note: It is now possible to mix framrates in Media Composer version 4.x and higher of the software. Even though it is possible to mix framerates in the timeline, the deck's output must match the project's format when capturing.
